Epilepsy as a dynamical system, a most needed paradigm shift in epileptology

چکیده
The idea of the epileptic brain being highly excitable and facilitated to synchronic activity has guided pharmacological treatment since early twentieth century. Although tackling epilepsy's seizure-prone feature, by tonically modifying overall circuit excitability and/or connectivity, last 50 years drug development not seen a substantial improvement in seizure suppression refractory epilepsies. This review presents new conceptual framework for epilepsy which temporal dynamics disease plays more critical role both its understanding therapeutic strategies. repetitive epileptiform pattern (characteristic during ictal activity) other well-defined electrographic signatures (i.e., present interictal period) are discussed terms sequential activation motifs. Lessons learned from physiological neural circuitry used further corroborate argument explore transition proper function state instability. Furthermore, explores how interfering temporally dependent abnormal connectivity between circuits may work as approach. We also use probing stimulation access network evaluate power determine transitional states dynamical system it moves towards regions instability, especially when conventional monitoring is proven inefficient. Unorthodox cases, with little or no scalp correlate, ictogenic spread restricted neurovegetative, cognitive, motivational areas shown possible explanations sudden death (SUDEP) psychiatric comorbidities. In short, this paradigm shift way that we address aimed encourage debate rather than narrow rationale currently engaged in.This article part Special Issue “NEWroscience 2018".

The aim of this report is to briefly comment on the current status of exercise genetics and genomics and to s...
متن کاملA review on the most valuable biomarkers in epilepsy
Background & Objective: Epilepsy is a common neurological disorder with the prevalence of 50 million in the world. Despite of many researches and introducing of different anti-epileptic drugs, 30% of patients suffer from refractory seizures. Biomarkers are the measurable indicators to detect the physiological and pathological condition. Biomarkers in epilepsy can be utilized for prediction, dia...
متن کاملA Methodology for Paradigm Shift to Explain Sustainable University
The sustainable university concept is rooted in prevalent paradigms pertained to economic, social and environmental development, which all focus on systemic thought and rationality; besides, they tend to discover and solve problems by applying an interdisciplinary approach. However, the epistemic component is what these paradigms have ignored. The purpose of this study is to explain a methodo...
متن کاملEpilepsy treatment: a paradigm shift is urgently need.
Epilepsy affects between 5 and 10 people in a 1,000 and carries considerable morbidity and premature mortality. The complex inheritance pattern of a lowered seizure threshold is not fully understood but is likely to be polygenic. In the majority of people with epilepsy, we do not understand the pathophysiology, how a seizure is triggered, and how it can be prevented. In the centennial year of t...
